That’s why we created the Care Plan System—our way of providing structured, personalized support that truly makes a difference. As part of our broader Comprehensive Support model, the Care Plan System ensures that every family receives the right help at the right time.

Rather than offering a one-size-fits-all solution, we walk alongside families, helping them move from crisis to self-sufficiency with care, dignity, and purpose.

How the Care Plan System Works

When a family enters our program, they aren’t just handed a few resources and sent on their way. We work closely with each household to assess their situation and match them with the appropriate level of support. It’s about prioritizing needs efficiently while ensuring no one falls through the cracks.

Families are placed into one of three support levels based on the severity and complexity of their circumstances.

Level 1: Short-Term Assistance (3–9 Months)

Families in Level 1 are typically facing a temporary setback—like a job loss, a medical emergency, or a housing disruption. They are otherwise stable and simply need a short boost to regain footing.

Support at this level includes:

  • Immediate crisis intervention to meet urgent needs
  • Financial counseling to help families reorganize their budgets and expenses
  • Short-term food assistance to relieve immediate pressure

The goal is to help these families recover quickly and avoid slipping into long-term instability.

Level 2: Mid-Term Support (9 Months–2 Years)

Families in Level 2 need deeper, sustained assistance to overcome multiple challenges. They might be battling childcare barriers, job loss, unstable housing, or a combination of factors that threaten long-term security.

Services offered include:

  • Access to job training and employment resources
  • Help enrolling in affordable childcare programs
  • Mental health counseling and emotional support services
  • Regular food and economic assistance to meet basic needs

Our aim with Level 2 families is to provide a strong foundation for lasting stability.

Level 3: Long-Term Support (Multiple Years)

Level 3 families often face generational struggles that require long-term solutions. This group may include grandparents raising grandchildren, individuals coping with chronic illnesses, or families entrenched in multigenerational poverty.

We support Level 3 families with:

  • Comprehensive case management to coordinate multiple services
  • Ongoing housing and financial aid support
  • Educational mentorship programs for children
  • Long-term food security support through consistent deliveries

For these families, our role isn’t just to provide services—it’s to build an enduring partnership.

Each family’s journey is carefully tracked through a letter and number system that helps us monitor progress and adjust care as needed:

  • A: Short-term support (under 3 months)
  • B: Mid-term support (3–9 months)
  • C: Mid-long term support (6 months–2 years)
  • D: Long-term support (2+ years)

By personalizing every Care Plan, we ensure that families aren’t overwhelmed—or under-supported—at any point in their journey.
